Please arrive about 10 minutes early so that you have enough time to fill out a new patient form, or do it ahead of time and bring it with you using the form below.

Based on your history and the completed dental examination, your dentist will develop a long-term treatment plan for you. Your dentist will also take the time to answer any questions you have about your oral health and treatment options.
